New Patient Registration

As of January 2015 GP practices are able to register patients from outside their practice boundary.  Here at James Alexander Family Practice we are happy to consider applications to join our panel from anyone.  Patients wishing to register are required to complete a registration pack which will include a health questionnaire to provide useful information whilst we wait for your medical records to arrive from your previous doctor, prior to being seen.  These can be found on our website, or are available from our reception.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
